Gonadal status in males with acute coronary syndrome and the extent of coronary artery disease: a comparative cross-sectional analysis

Bednarova A, Koller T, Kuzma M, Cenkerova K, Malacky T, Payer J

Cardiology Letters 2018, 27(2):87-93

Introduction and aims: Association between coronary artery disease (CAD) and male gonadal status has been previously suggested, but still remains inconclusive. The aims of our study in males with recent acute coronary syndrome (ACS) were to evaluate the prevalence of hypogonadism, to compare their gonadal status with non-ACS matched controls and to evaluate gonadal status by the extent of CAD. Subjects and methods: A cross-sectional retrospective study included 96 males with recent ACS and age and 49 BMI matched non-ACS controls. Basic demography and standard CAD risk factors were recorded as well as fasting serum levels of total testosterone (TT),...

Ischaemic heart disease - which way can treatment of hypertension help us here?

Murín J

Cardiology Letters 2018, 27(2):94-98

Ischaemic heart disease (IHD) is the most common disease in clinical practice with the highest morbidity and mortality. Arterial hypertension is one of most frequent risk factors for IHD together with dyslipidemia. Prevention of development/progression of IHD with good treatment of cardiovascular risk factors, mostly hypertension, is a good step in improving the cardiovascular prognosis of these patients. This article discusses pathways of IHD development through hypertension, and the pathways of development in ischaemic events. It tries to show us also the best treatment possibilities in patients with hypertension and ischaemic heart disease together....

First implantation of the total artificial heart in Slovakia in patient with critical biventricular heart failure

Gonçalvesová E, Lesný P, Olejárová I, Hulman M

Cardiology Letters 2018, 27(2):99-105

End-stage heart failure is a public health problem affecting an increasing number of patients each year. The lack of donors is leading to increased use of ventricular assist devices (VAD) as a bridge to transplant (BTT). Among patients with advance heart failure, those with biventricular failure have the worst prognosis. Implantation of a total artificial heart (TAH) is an alternative to durable biventricular VAD support providing possibility of ambulatory care. In this report we present our experience with the implantation of the Syncardia TAH, the first in the area of Eastern Europe (worldwide 1741), in a patient awaiting transplantation with critical...

Early graft thrombosis following CABG due to antithrombin-III deficiency

Bezak B, Hudec V, Artemiou P, Hulman M

Cardiology Letters 2018, 27(2):106-109

Early graft failure after coronary bypass grafting is a serious complication which may result in acute myocardial ischemia. In this report we present the case of a 79-year-old female patient with repeated multiple graft thrombosis of initially unclear origin which, after further investigation, was most likely caused by acquired antithrombin III deficiency. Although antithrombin III deficiency is a rare complication, in the case of simultaneous acute grafts thrombosis after cardiac revascularization surgery, it should be considered as a probable cause.

ODYSSEY OUTCOMES. clinical study: Implications for us

Murín J, Čaprnda M, Kasperová V

Cardiology Letters 2018, 27(2):76-86

Patients suffering acute coronary syndrome (ACS), even with optimal treatment of ACS and optimal secondary prevention treatment afterwards, still have a very high cardiovascular (CV) risk of morbidity and mortality. More intensive treatment of dyslipidemia, with further reduction of serum LDL-Ch levels, to improve the prognosis of these patients - was the hypothesis of this study, with alirocumab (PCSK9 inhibitor) treatment versus placebo treatment. There were 18 924 patients in this study, randomized for the mentioned treatment in the time period of 1 - 12 months after ACS. In the active arm treatment there was a further reduction of serum LDL-Ch...
