Perspectives of anticoagulation therapy in cardiovascular medicine

The two last decades have witnessed a revolution in the field of anticoagulation. Direct oral anticoagulants (i.e., factor IIa- and factor Xa- inhibitors) progressively dominate clinical practice because of their favourable pharmacological profile and ease of use, particularly in venous thromboembolism (VTE) treatment and stroke prevention in atrial fibrillation. However, the residual risk of cardio- and cerebrovascular events, particularly in some critical settings, and the risk of major bleeding still represent unmet medical needs. As a result, ongoing research focuses on developing future anticoagulants with an improved safety profile. Several promising approaches to reduce the bleeding risk involve targeting the intrinsic (or contact activation) pathway of coagulation, with the goal of preventing thrombosis. Based on epidemiological data on hereditary factor deficiencies and preclinical studies factors XI and XII emerged as the most promising candidate targets. This review summarizes current knowledge on FXI and FXII inhibitors, with a particular focus on their pharmacological properties and potential clinical indications.

Záver Hlavným cieľom použitia inhibítorov FXI a FXII je uspokojenie tých terapeutických cieľov, ktoré súčasné DOAK-y nepokrývajú a tým vyplnenie medzier v širokej škále možných terapeutických indikácií. Iniciálne iste nebude cieľom nahradiť súčasné DOAK-y, ale poskytnúť lieky pre tie klinické situácie, v ktorých sú DOAK-y kontraindikované alebo v ktorých ich benefity neboli spoľahlivo preukázané. V tejto súvislosti môžu byť najdôležitejšími indikáciami prevencia závažných nežiaducich kardiovaskulárnych príhod u pacientov s terminálnym ochorením obličiek/hemodialýzou a prevencia trombózy, ktorá súvisí s dočasne alebo trvale zavedenými intravazálnymi implantátmi, ako sú centrálne venózne katétre, mechanické chlopňové protézy a zariadenia na podporu funkcie srdcových komôr. Vysoko účinná antikoagulačná stratégia, ale s lepším bezpečnostným profilom, bude osobitne dôležitá u pacientov s vyšším rizikom krvácania, ako sú pacienti s nádorovým ochorením alebo pacienti s fibriláciou predsiení, ktorí sú pre aterosklerotické komplikácie kandidátmi na duálnu alebo trojitú anti­ trombotickú terapiu. Ambicióznejším cieľom bude nahradiť DOAK-y pri minimálne rovnakej účinnosti, ale pri lepšom profile bezpečnosti, napríklad v širšej populácii pacientov s fibriláciou predsiení. Ďalšími potenciálnymi indikáciami môžu byť akútne koronárne syndrómy a ischemická NCMP, kde v súčasnosti stále neexistuje konsenzus o používaní perorálnych antikoagulancií, respektíve je ich používanie obmedzené. Prebiehajúce alebo plánované klinické štúdie fázy 3 budú musieť poskytnúť všetky potrebné dôkazy, ktoré sa týkajú
