Combination of ACE inhibitor perindopril-arginine and calcium channel blocker amlodipine: more than only a reduction of increased blood pressure

The present management of hypertension focuses not only on blood pressure (BP) values, but also on additional cardiovascular (CV) risk factors. Considering total CV risk in addition to BP levels is aimed to maximize effectiveness of the therapy since in high-risk individuals antihypertensive treatment strategies may be different from those to be implemented in lower-risk individuals. The 2013 European Society of Hypertension/European Society of Cardiology (ESH/ESC) guidelines on hypertension management now advocate combining two antihypertensive agents for the initial treatment of patients at high CV risk. Also they acknowledge the benefits of simplicity and better compliance with antihypertensive fixed-dose combinations. Those combinations that have been successfully used in trials should be preferable. One two-drug combination recommended by the ESH/ESC is an angiotensin converting enzyme (ACE) inhibitor/calcium channel blocker (CCB), such as perindopril/amlodipine. The efficacy and safety of perindopril-arginine/amlodipine combination is well established. The best evidence comes from outcome studies with perindopril/ amlodipine combination, particularly the ASCOT trial. Moreover, the perindopril/amlodipine combination therapy can reduce the risk of developing resistant hypertension. In a real-life practice setting, further effective, safe and sustained 24-h ambulatory BP reductions were obtained with the perindopril/amlodipine fixed-dose combination in hypertensive patients uncontrolled on ACE inhibitor and/or CCB.
