Impact of left ventricular systolic dysfunction on the right ventricle in terms of three-dimensional echocardiography

There is a need to assess the right ventricle in the diagnosis of chronic heart failure, because its function is proved to be an important indicator of prognosis, exercise tolerance and quality of life for the failure of the left ventricle.

Aims: The target was to investigate the relationship between systolic dysfunction of the left ventricle (LV) and right ventricle (RV) by three-dimensional echocardiography.

Methods: Off-line analysis of echocardiographic results were obtained in 55 patients (42 men, 13 women, mean age 62,6 ± 11,8 years) using three-dimensional echocardiography (3DE) and two-dimensional echocardiography (2DE) with LV systolic dysfunction (ejection fraction < 50%). Determined were parameters of systolic and diastolic LV function, volume and shape of the LV, RV volume and function, pulmonary artery acceleration time and presented quantified tricuspid regurgitation, all by software off-line analysis.

Results: We noticed a correlation between right ventricular ejection fraction (RV EF) and LV mass indexed to body surface area (LV MASS/BSA) (p = 0.02). Other echocardiographic parameters in relation to RV EF were proved to be statistically insignificant. We observed the relationship between the NYHA functional group and RV EF (p = 0.046). There was no correlation between NYHA and LV EF (p = 0.493). Statistically significant was the relationship between the value of nocturnal dyspnoea and RV EF (p = 0.014) and no correlation with LV EF (p = 0.99).

Conclusion: We have shown a statistical dependency between RV EF and LV MASS/BSA, GLS and PV AT. NYHA functional classas nocturnal dyspnoea data in patients with LV systolic dysfunction was dependent on the RV EF.
