Microvascular angina pectoris (cardiac syndrome X) and its relation to insulin resistance and metabolic syndrome

In the examined group of 500 patients subjected to coronary catheterization, 70 cases (14%) presented a "normal" coronary angiogram. This type of angina pectoris is referred to as microvascular angina pectoris (MIV-AP) or cardiac syndrome X (CSX). We detected a prevalence of periclimacteric women with android obesity (4,1 times): in our sample 52% of them showed symptoms of benign hirsutism and suffered from long-term menstrual cycle disorders. In addition, half of of the MIV-AP patients presented in the myocardial SPECT thallium scintigraphy a significant myocardium perfusion defect during a repeated ergometry. In our study of MIV-AP patients, no elevated levels of endothelin-1 (ET-1) were detected in comparison to healthy population or to patients with significat coronary stenosis that could be used as a relevant MIV-AP marker. On the other hand, we were able to prove the presence of insulin resistance by means of four tests (HOMA, FIRI, Quicki and our indirect insulin sensitivity index-NISI) and phenomena of metabolic syndrome or of syndrome of insulin resitence.
