Background: Potential of using the T-peak to T-end (TpTe) interval as an electrocardiographic parameter reflecting the transmural dispersion of ventricular repolarization (TDR) to identify patients (pts.) with higher risk of malignant ventricular arrhythmias (MVA) for better selection of candidates for implantable cardioverter-defibrillator (ICD) in primary prevention (PP) of sudden cardiac death (SCD) remains controversial. Objective: The primary objective of this study was to investigate the relationship between the TpTe interval on a resting 12-lead electrocardiogram (ECG) and the incidence of MVA resulting in appropriate ICD intervention (AI)....
CLARIFY registry - five-years follow-up of patients with chronic coronary syndrome
Murín J
Cardiology Letters 2020, 29(2):93-96
Coronary artery disease (CAD) is one of the most common and serious cardiovascular (CV) diseases in the developed countries. Last year there was a change in the classification of CAD presented at the ESC Congress, 2019. The dynamic nature of the CAD process results in various clinical presentations, which can be categorized as either acute coronary syndromes (ACS) or chronic coronary syndromes (CCS). Stable CAD has been renamed to Chronic Coronary Syndrome (CCS). CLARIFY registry (32 703 patients with CCS from 45 countries) was a prospective observational registry (starting Nov 2009 - Jun 2010 with 5-years follow-up) and its data were analyzed: a)...
Polypill in seniors: possible frequency of fixed drug combination according to the SAFIS study
Dúbrava M, Németh F, Drobná T, Gerlich L
Cardiology Letters 2020, 29(2):97-101
Aims: Polypharmacy is one of the most typical features of geriatrics. Unreliable drug usage belongs to its risks, among others. One way of reducing this risk is to integrate more drugs into one tablet (polypill). Nowadays this could be done especially in cardiovascular medicine. Real world data from the health care provided in Slovakia, which would reflect the potential for switching from more tabs with one drug towards the polypill in seniors, are almost unknown. Methods: We used the "Slovak Audit of atrial FIbrillation Study" (SAFIS) data (3 706 patients, average age 80.5 years) and evaluated how frequently patients had the combination of an angiotensin...

Indication and importance of ABPM in clinical practice
Gašpar Ľ, Filipová S
Cardiology Letters 2020, 29(2):102-105
Arterial hypertension is one of the most important risk factor for cardiovascular diseases, including ischemic and haemorrhagic stroke, coronary heart disease, heart failure, and also dementia, vision loss and kidney failure. Risk of cardiovascular complications increases continuously with blood pressure. Indication categories of ambulatory blood pressure monitoring (ABPM) are diagnostic, therapeutic and prognostic. In addition to the standard parameters of blood pressure, ABPM enables monitoring of its circadian variability with the determination of diurnal index. It provides also indispensable information about the effectiveness of drug therapy in...
Cardiac mitochondria in the role of cardioprotective strategies
Andelová N, Jašová M, Kancirová I, Ferko M
Cardiology Letters 2020, 29(2):106-112
In recent years, the view of cardiac mitochondria as an energy source providing the ATP supply needed to maintain contractile function, basal metabolic processes and ionic homeostasis has changed significantly. It is currently known that dysfunction of these organelles results in the development of a large number of diseases, including cardiovascular diseases. Mitochondria are currently considered a promising target and end effector of endogenous strategies necessary to protect the myocardium from acute ischemia/ reperfusion injury. The present review offers an overview of survival cascades involved in the endogenous protective mechanisms and adaptability...

Effect of sub-chronic administration of pioglitazone on blood pressure and endothelial function in borderline hypertensive rats: the role of nitric oxide
Kvandová M, Bališ P, Púzserová A, Dovinová I
Cardiology Letters 2020, 29(2):113-120
Aim: Peroxisome proliferator-activated receptor gamma (PPARγ) is a ligand-dependent intracellular protein and interest in its activation has risen over recent years, considering its influence on hypertension development. The aim of this study was to investigate the effect of sub-chronical activation of PPARγ by the exogenous agonist - pioglitazone on blood pressure, activity of nitric oxide synthase (NOS), superoxide production (O2*-) and on the functional status on the endothelium of small resistant arteries in experimental borderline hypertensive rats (BHR). Methods: Young (5 weeks) and adult (12 weeks) male borderline hypertensive rats...

Health-care workers and patient focused proposal for anti-epidemic management during the COVID-19 epidemics in Slovakia
Hatala R
Cardiology Letters 2020, 29(2):73-77
The aim of the presented proposal is to counteract the dramatic spread of the COVID-19 disease and to target reduction of its high case fatality rate by risk stratification during the early phases of the disease. Two key factors have contributed to the catastrophic extent of the disease in several countries: - Most of the known COVID-19 hotspots were locally preceded by massive gatherings of up to several hundred-thousand people (sporting events, religious pilgrimages etc.). - Widespread, mostly subclinical infection of health care workers (HCW) in the hospitals of the hotspot regions inabled a crucial dramatic virus transmission to elderly population....
