Managing cardiotoxicity induced by chemotherapy in centers from low income countries

Rodríguez-Ramos MA, Fardales-Rodríguez R

Cardiology Letters 2019, 28(1):21-25

Cardiovascular diseases and cancer account for 60% of deaths in the Western world. They have common risk factors, such as obesity and diabetes, and are often found in the same patient. It is more common in a patient with tumour disease who develops cardiovascular involvement secondary to treatment with cytostatic or radiotherapy. A protocol of in-hospital management of cardiotoxicity induced by cytostatic is proposed with the objective of providing specialized care to patients with cancer and a history of cardiovascular disease or who develop cardiac complications during cancer therapy, in centres of low-income countries. Care and follow-up criteria...

Risk stratification in pulmonary artery hypertension: the way to better outcomes

Luknár M, Lesný P, Gonçalvesová E

Cardiology Letters 2019, 28(1):26-32

Pulmonary arterial hypertension (PAH) is a rare disease affecting small pulmonary arteries. It results in right ventricular pressure overload and failure. Despite the availability of new targeted pharmacologic treatment, PAH remains an incurable disease. The disease course is variable and it is thus necessary to identify patients with unfavourable prognosis. New tools for risk assessment both at initial presentation and at periodical examination have recently become available. They integrate individual risk markers. The aim of therapeutic measures is to achieve and maintain the low risk status. If this goal is not achieved, treatment escalation is...

How is the treatment of diabetic patients changed to reduce cardiovascular risk?

Murín J, Bulas J, Wawruch M

Cardiology Letters 2019, 28(1):33-40

Diabetes mellitus type 2 is an important and serious risk factor of cardiovascular (CV) disease which is here the main reason of mortality in diabetic patients. In the last three of four years the results have been published of large clinical studies with SGLT2 inhibitors and GLP-1 receptor agonists, which have shown a great CV benefit. SGLT2 inhibitors have favourable metabolic, hemodynamic and renal effects which explain their great CV benefit. GLP-1 agonists favourably influence body weight, blood pressure, serum lipids and renal function, but their CV benefit comes later than that from SGLT2 inhibitors treatment. Large clinical studies (EMPA-REG...

Bystander CPR and successful Primary Percutaneous Coronary Intervention

Bulíková T, Cmorej PCH, $4

Cardiology Letters 2019, 28(1):41-45

The authors present a commented case report of out-of-hospital cardiac arrest with initial defibrill ated rhythm as a deterioration of acute myocardial infarction. Early detection of STEMI and initialisation of rapid transport to PCI depends on several factors, such as the early start of lay CPR provided by bystanders, early defibrillation and achieving of ROSC by the EMS crew, and 12 leads ECG provided soon after ROSC. The outcome is higher if the time between call to Emergency Medical Dispatching and handover in the hospital is less than 90 minutes.
