Necroptosis-mediated damage of atrial myocardium - another proarrhythmic factor of atrial fibrillation?

Atrial fibrillation (AF) is the most common heart rhythm disorder worldwide, with a dominant position in the epidemiology of cardiovascular diseases. In spite of a number of various therapeutic approaches for the restoration and/or maintenance of sinus rhythm, the mortality of patients with AF is twice as high as in patients with sinus rhythm. This suggest that other, so far unknown, factors are likely responsible for atrial damage, and that targeting such factors might be more beneficial in the management of AF. It is known that besides electrical and contractile remodelling, structural remodelling of atria, particularly due to inflammation-related fibrosis, also plays an important role in the complex pathophysiology of AF. Inflammatory response is also closely associated with recently discovered necroptotic cell death, which is defined as a RIP3-MLKL-driven cell death modality with a necrosis-like phenotype. It has been shown that necroptosis is implemented in the pathogenesis of ischemic heart disease as well as heart failure. In this study, we present a hypothesis about a plausible role of necroptosis in the atrial damage and its linkage with inflammation which may result in AF. The individual premises of our hypothesis are based on the results of a pilot study and on the basis of them we indicate future direction of research in this field of cardiology.
