Predsieňové tachyarytmické epizódy v klinickej praxi

Záchyt predsieňových tachyarytmických epizód (AHRE epizódy), ako aj subklinickej fibrilácie predsiení (SCAF) pomocou implantovaných elektroimpulzogenerátorov rastie úmerne s dĺžkou sledovacieho obdobia pacientov. Nálezy AHRE epizód pri bežných arytmologických kontrolách elektroimpulzogenerátorov značne narastajú. Takmer 35 % pacientov s implantovaným kardiostimulátorom má počas sledovania 2,5 roka aspoň jednu AHRE epizódu. Význam a riziko daných epizód sa neustále skúma a vyvíja v čase. Podľa aktuálnych znalostí vieme, že AHRE epizódy predstavujú nielen riziko vzniku fibrilácie predsiení, ale aj samostatné nezávislé riziko vzniku cievnej mozgovej príhody a taktiež sa spájajú s nárastom kardiovaskulárnej morbidity i mortality. Holistický prístup k AHRE epizódam zdôrazňuje významnosť fibrotického procesu ako hlavnej spúšťajúcej príčiny vzniku tzv. fibrotickej atriálnej kardiomyopatie a súvisiaceho tromboembolizmu.
