Hodnotenie subklinickej systolickej dysfunkcie ľavej komory pomocou parametrov deformácie

Dobrá ejekčná frakcia neznamená vždy dobrá systolická funkcia srdca. Ejekčný mechanizmus srdca je zložený z viacerých pohybov, pričom práve longitudinálna deformácia je pri rôznych ochoreniach poškodená ako prvá. Takéto začiatočné poškodenie longitudinálnej kontrakcie nie je pri echokardiografickom vyšetrení viditeľné voľným okom. Na jeho detekciu spoľahlivo slúži metóda nazývaná speckle tracking. Za ostatných 15 rokov sa táto metodika štandardizovala, získali sa referenčné hodnoty a postupne nachádza aj klinické uplatnenie pri situáciách, kedy je nevyhnutné posúdiť, či už nedošlo k subklinickej deteriorácii systolickej funkcie. Ide najmä o asymptomatickú závažnú aortálnu stenózu, oblasť kardioonkológie alebo rôzne stavy spojené s hypertrofiou stien myokardu. Speckle tracking bude onedlho úplne štandardnou výbavou prístrojov, preto okrem oblastí jej uplatnenia je vhodné zdôrazniť aj niektoré praktické aspekty pri získavaní dát na takúto analýzu.
