Východiská a princípy efektívnej liečby diuretikami pri srdcovom zlyhávaní

Diuretiká sú kľúčovou a pravidelnou súčasťou liečby symptomatických pacientov so srdcovým zlyhávaním bez ohľadu na systolickú funkciu ľavej komory. Cieľom podávania diuretík je odstránenie kongescie, ktorá je nielen dominantou príčinou symptómov a príznakov, ale aj dôležitým faktorom nepriaznivej remodelácie myokardu a progresie jeho dysfunkcie. Pacienti s pretrvávajúcimi alebo rýchlo recidivujúcimi príznakmi kongescie majú zlú prognózu, súčasne však sa vyššia mortalita pozorovala aj u chorých s vysokými dávkami diuretík. Princípom úspešnej a bezpečnej liečby diuretikami je udržanie optivolemického stavu pri použití čo najnižších dávok. Praktickú realizáciu tohto princípu obmedzuje nízka senzitivita klinického vyšetrenia pri detekcii kongescie, variabilita intenzity kongescie, ako aj účinok diuretík. Dávkovanie diuretík má byť preto flexibilné s pravidelnou kontrolou ionogramu a renálnych funkcií, predovšetkým pri zmene bazálnej dávky diuretík, ACE inhibítorov alebo antagonistov aldosterónových receptorov. Prehľadný článok sa venuje mechanizmu účinku, hemodynamickým a metabolickým efektom diuretík a ich jednotlivých tried. Rozoberá názory a dôkazy o vplyve diuretík na prežívanie pacientov so srdcovým zlyhávaním a perspektívy optimalizácie ich používania. Fig. 2, Tab. 1, Lit. 44, online full text (Free, PDF)
