What is new in patients with HFpEF?

Heart failure with preserved ejection fraction is common in the population of patients with heart failure syndrome and treatment is difficult. Whereas heart failure with a reduced ejection fraction can be treated with drugs that act to attenuate the overactivation of endogenous neurohormonal systems, therapeutic options for patients with heart failure and a preserved ejection fraction are limited. EMPERORPreserved (The Empagliflozin Outcome Trial in Patients with Chronic Heart Failure with Preserved Ejection Fraction) was carried out to evaluate the effects of SGLT2 inhibition with empagliflozin on major heart failure outcomes. Treatment with empagliflozin significantly reduced the combined risk of cardiovascular death or hospitalization for heart failure in patients with heart failure and a preserved ejection fraction, regardless of the presence or absence of diabetes. This is the first evidence of efficacy of drug treatment for heart failure with preserved ejection fraction.
