The authors present a commented case report of out-of-hospital cardiac arrest with initial defibrill ated rhythm as a deterioration of acute myocardial infarction. Early detection of STEMI and initialisation of rapid transport to PCI depends on several factors, such as the early start of lay CPR provided by bystanders, early defibrillation and achieving of ROSC by the EMS crew, and 12 leads ECG provided soon after ROSC. The outcome is higher if the time between call to Emergency Medical Dispatching and handover in the hospital is less than 90 minutes.
