New treatments for dyslipidemia

Cardiovascular diseases are the main reason for mortality and morbidity in the world, and also in our country. An important risk factor for cardiovascular disease is dyslipidemia, mainly hypercholesterolemia. A reduction of serum LDL cholesterol level (by diet but mainly by pharmacotherapy) decreases cardiovascular risk of these patients. For the reduction of serum LDL cholesterol levels we use usually statins (mostly strong statins and used in higher doses), ezetimibe and their combination. New drugs are in the pipeline and just entering clinical practice. Among these drugs are ApoB production inhibitors (mipomersen), a microsomal transfer protein activation inhibitor (lomitapib), drugs supporting lipoprotein catabolism (mainly PCSK9 inhibotors) and some other drugs, for example ETC-1002. Another possibility is an increase of serum HDL cholesterol level and we are here waiting for results of clinical studies with anacetrapib and evacetrapib. There is also some clinical research to influence positively lipoprotein Lp(a). Where we are now, and what direction hypolipidemic treatment will follow, is the main message here.
