Hodnotenie antikoagulačnej aktivity a zvrátenie antikoagulačného účinku nových perorálnych antikoagulancií - aké máme možnosti?

Nové perorálne antikoagulanciá (NOAC) predstavujú významný krok dopredu pre pacientov s fibriláciou predsiení. Poskytujú možnosť podstatne znižovať morbiditu a mortalitu v dôsledku kardioembolických mozgových príhod. Tieto lieky, ktoré v súčasnosti zahŕňajú priamy inhibítor trombínu dabigatran a priamy inhibítor faktora Xa rivaroxaban, poskytujú pacientom množstvo výhod, bez potreby laboratórneho monitorovania. Bezpečná preskripcia NOAC si však vyžaduje primerané poznatky o ich farmakológii, stanovení funkcie obličiek, vedomosti o účinku týchto liekov na bežne používané koagulačné testy, dostupnosť presných laboratórnych testov na stanovenie antikoagulačného účinku NOAC v špecifických klinických situáciách a ovládanie reverzných stratégií, ktoré možno použiť v naliehavých prípadoch (u pacientov s krvácavými komplikáciami alebo vyžadujúcich urgentný invazívny výkon). Súčasné použitie dabigatranu alebo rivaroxabanu a kyseliny acetylsalicylovej alebo iných protidoštičkových liekov sa spája so zvýšeným rizikom krvácania, ktoré je podobné riziku pozorovanému pri warfaríne. Ich súčasné používanie sa musí starostlivo posúdiť, pričom treba vziať do úvahy individuálny rizikový profil.
